One of the issues appears to be that characters who have been locked out in the past become bugged and remain stuck, resulting in an error message stating that a character has already logged into the game. One of the recommendations made by Blizzard is to avoid using the same character names for both offline and online characters, as this is thought to be related to the character lock problem.
